DeQuervain’s Tenosynovitis

What is it? 

  • This is a condition that affects the tendons on the outer part of the thumb and thumb side of the wrist.

  • It is commonly seen in mothers of new babies, either due to hormonal changes and/or repetitively lifting a new baby.

  • However, this condition is not exclusive to new mothers and can affect anyone. It can be aggravated by activities that use the hands/wrists such as gardening, lifting, housework or sport. 

 

Typical Symptoms/features:

  • Pain over the outside of the thumb or thumb side of the wrist which can be painful to press.  

  • May have swelling in the area.

  • Pain on thumb movements, especially when lifting the thumb into a hitchhiker’s position or when using scissors. 

  • Pain on resisted thumb movements.

  • Pain on gripping and lifting tasks.

  • May hear clicking or snapping of the tendon. 

 

Management:

  • Activity modification 

- When you first start noticing pain, it may help to reduce or modify your activity for a time to allow the pain to settle.   

  • Ice or heat.

  • Pain relief.

  • Physiotherapy including range of movement exercises and a graded strengthening programme.

  • Using a wrist/thumb splint may be useful.

  •  

    Orthopaedic options: In cases with severe and/or prolonged symptoms, options such as corticosteroid injections or a surgical opinion may be warranted.  

 

Do I need imaging?  

  • Imaging (X-rays or scans) is rarely required to diagnose DeQuervain’s.

  • X-rays and scans are most appropriately used after a serious injury.
